That’s part of why TFR in places like Italy & Japan is so low (~ 1.4), while it’s 6.7 in Niger. Because in the Sahel having kids take care of you in old age is the only retirement plan they have. So, if 1/3 of your kids die in childhood (and another 1/3 die in early adulthood), people intuit that they better have “extra”. So, creating conditions for kids to survive childhood will bring down the birth rate. At least that’s the assumption. Gates can be trying to save lives to reduce global population.
