I don’t know if you know it, but when I was 19, I flitted off to Houston, Texas for a summer job that lasted about three years until the oil boom busted.
It is one of the things our old coyote pal and I had seen about the same time. I lived in Pasadena, not Houston, because it was cheap and I was such a country bumkin I thought Mexicans were exotic like Europeans. I got myself in a lot of trouble when I was in Houston, but I managed to avoid the Houston Hundred, unlike a lot of the women in this video.

A big part of the answer is in the Food Inc. documentary that you posted a while ago (a few months ago, last year maybe?). Relatedly, the crap processed food in America wrecks the delicate balance in the gut between good and bad bacteria. When out of balance, it feeds the craving for more unhealthy food (sugar, carbs) — so it is a destructive cycle.
So I have some sympathy for those that end up obese, but there is enough understanding of its causes and how to remediate it now, that it comes down to the person’s desire to change. Many don’t want to — this fat lady shown thinks she looks good, so if you believe she belives that, then see won’t do anything except die early.

They focus on Houston but, in reality, most of America is fat/borderline obese. Once upon a time people walked to get somewhere, they worked manually and that work was anything from cleaning house to tossing hay. The shift from rural to urban brought with it a change in lifestyle and working. Cars became the major means of transport and sitting behind desks became work.
I look at people in gyms, riding their stationary bikes and running on treadmills, moving but going nowhere, as a great illustration of modern life.
They did touch on one fact about Houston and most of Texas…the weather. Beastly hot in summer. Without AC Texas is uninhabitable. I’m up early, often at dawn, to work outside, by 11:30 it’s already getting miserable and I hide until five to go back outside. I love good BBQ, why those guys were so fat is because…beer. They eat salty greasy meat and wash it all down with lots of beer.
One other fact here…processed food is fattening and….addictive. Sugar, salt and grease, no nutrition but the body craves them the same way it craves heroin. The same neural receptors are at work here
